Make sure you are ready to pay at the time of service for sessions.

I am only accepting referrals for children who can make daytime appointments and are private pay, Providence, Regence, and Moda.

If you have another insurance plan, we can provide you with a Superbill to submit to your insurance (Medicare plans do not accept out of network charges, only private health plans).

Step Two

Complete the Client Interest form

Complete this form, and we will contact you when there is an opening.

In your first session, an evaluation will be completed to decide what your child's needs are, and to create a treatment plan.

At this time, we are unable to offer clinic tours or free phone calls with Sarah, as she is fully booked with sessions during the day.
